Use the Fidelity "Transfer of Assets" Form.

Put "Oakmark Funds" as "Delivering Firm Name."

Check "Traditional IRA" on ACCOUNT TYPE and ACCOUNT TYPE BEING TRANSFERRED.

"Name of Fund" should be OAKBX (Oakmark Equity Income).

There will be a nominal transfer and/or closing fee of around $20.00 by Oakmark.
