Add Verizon to your list. Their customer service is the worst!
They couldn't keep their appointments for months when I switched to them from Vonage. (I didn't enjoy losing phone service whenever the electricity went out, or if my computer was malfunctioning.)
Then I subscribed to Verizon's FIOS package that includes phone, Broadband internet, and TV.
The product and the package is good, but don't waste time trying to talk to anyone in their customer service department.
